How do they come loose after a while if it's coupled with a screw?
I normally use a little dab of glue when installing but that's just so that it does not shift when trying to thread the screw.

if you hammer them in, sometimes the spikes bend instead of going in fully into the wood so you may just have 2-3 spikes holding instead of the 4

Best way to get them in is with a screw and tightening each one. I even used a G-clamp once and it worked out fine.

I personally find that T-nuts in MDF tend to rip out if you overtighten the bolt. The threaded insert sounds like something i wanna try.
