I hope not. I traveling Saturday and my passport expires March of next year. AA said it's not a problem and us consular services indicate once you staying within the validity of the passport you should be okay.

Hmm .. you may not have a problem .. 5 months valid.
You should verify with US Embassy in TT .

Yes. Please ensure that you print it on the correct type and size of paper (legal size, 20-24 lb. paper).

say, from a paper clip, will show up in the scan.

No. Just the documents/copies of documents listed in the link below.

Not sure. Only if you don't have your national identification card, then you should at least walk with your driver's permit as I assume that will be the ID number you would be quoting in the declaration of applicant section of the form.
